Placing an order:

Items you will need to place your first order:

- Festoni Fax Cover Sheet
- Sales Tax ID
- 6 Trade References (with Account numbers and fax numbers)
- Purchase Order Form
- 1/2 Deposit (see below)
- Minimum Opening Order requirement (see Terms and Conditions)

Please print out the Fax Cover Sheet and Purchase Order Form, fill them out, sign them, and fax both forms along with your Sales Tax ID # and reference information.

We will not accept any first time orders without the tax and reference information.
We will not accept the Purchase Order without a signature.

Your first order will require a half deposit. We accept Visa, MasterCard, American Express or check.

After your initial order you do not need to resubmit your tax and reference information.
